Optic nerve drusen
These are hyaline bodies which can calcify at the optic nerve head, and are seen during slit lamp fundus biomicroscopy. A particular characteristic is autofluorescence of the drusen – seen above – using the correct filters, there is no need for any fluorescent molecules to help identify them. Some patients with optic disc drusen may…

PHPV – Calcified subluxated cataract
Case of persistent hyperplastic primary vitreous, eye shorter than normal, thickened sclera, and a luxated cataract in resorption with vitreous in the anterior chamber. The loose cataract was causing chronic anterior uveitis and pain, and at different visits would be half in the anterior chamber and half in the vitreous cavity. As the video…

Tractional retinal detachment
Case of advanced proliferative diabetic retinopathy. Difficult and long surgery, patient had silicone oil implanted as a vitreous substitute.
-
Pigment epithelial detachment
an unusually large PED in a young male with central serous chorioretinopathy
-
Neovascularization of the disc
Proliferative diabetic retinopathy, NVD spreading through the posterior pole.

Macular hole surgery
Pre and post-op OCT of a macular hole surgery. The hole was successfully sealed in this patient, however he could not feel any improvement in visual acuity. That can sometimes be observed in long-standing macular holes or when there was previously enough photoreceptor dysfunction.
-
Avastin in Wet AMD
This patient had 10 monthly intravitreal injections of Avastin in the left eye. Interestingly there is an OCT from before the CNV was active.
-
Cataract and vitreous hemorrhage
The patient above had a visually significant cataract and vitreous hemorrhage. She has a chronic heart condition and is on dyalisis due to diabetic nephropathy and we decided to perform a combined cataract phacoemulsification with intraocular lens implant and pars plana vitrectomy.

Epiretinal membrane and macular folds
Patient with old nasal branch retinal vein occlusion and epiretinal membrane causing retina folds. Pars plana vitrectomy was indicated due to low vision. Using the endolaser, the affected quadrant was photocoagulated. Membrane Blue was used to stain the macular pucker and facilitate membrane peeling. Gas was implanted by the end of this surgery, with…

Vitreous hemorrhage
This patient had a chronic recurrent vitreous hemorrhage from proliferative diabetic retinopathy. His recent bleed came from neovascularization at the superior retina, panphotocoagulation was performed.

Adult vitelliform lesion in both eyes.
It has been interesting to me to order wet AMD genetic studies in these patients, samples collected from a cheek swab, and follow the lesion over time.
